I do hereby acknowledge and understand that I will be training at Ballard Jiu Jitsu.

I further acknowledge that I am familiar with the risks involved in physical activity including but not limited to Jiu Jitsu, wrestling, and strength and conditioning training.

I further acknowledge that I understand that such training can result in injury or harm to myself. I acknowledge and assume any such risk to my person.

In the event that I should sustain any injury to myself as a result of training at Ballard Jiu Jitsu, I hereby agree to hold harmless Jiu Jitsu Collective LLC, its owners and employees.

I agree to follow instruction from professors and coaches, and I agree to leave the facility if asked to do so by a member of the Ballard Jiu Jitsu staff.

COVID-19 can be spread easily and it is incumbent upon instructors and students to do our part to minimize the risk of exposure. I recognize that even with safety precautions in place, there is still a risk of exposure. I will agree to ensure I participate in safe practices to ensure the health of the community. It is important that each member of our community agree to the following.

I will not train if I feel any symptoms of being sick including, but not limited to; loss of smell, dizziness, fever, cough, shortness of breath, sore throat, vomiting or diarrhea.

I will not train if I have been in contact with any COVID cases in the last 14 days.

I will maintain excellent hygiene habits which include wearing only clean training clothing, washing\sanitizing hands upon entry and exit of the school, not sharing water bottles, and not sharing equipment.

If I feel symptoms, I will notify the instructor immediately.

Although all these precautions should minimize the risk of COVID-19 exposure, it is still possible that exposure could happen. In the case that exposure to COVID-19 happens, students will not hold Ballard Jiu Jitsu liable. We will all minimize the potential exposure by adhering to best safety practices.

I have read and fully understand the contents of this "hold harmless" agreement and execute same as my own voluntary act.
